DigiCare4You is a Horizon 2020 programme project, which aims to develop and implement a community-based, evidence-driven intervention using schools as the entry point. Building on the experience of the Feel4Diabetes study, the project will include a two-step screening procedure to identify families at high risk of developing Type 2 Diabetes (T2D) and/or hypertension.

Following this phase, an intervention will be carried out that combines the use of digital tools with activities that support family well-being in schools and community.

The DigiCare4You solution takes into account the differences among health systems across Europe and, with a particular focus on vulnerable and underrepresented groups, will be implemented in two middle-income countries (Albania and Bulgaria) and two high-income countries (Greece and Spain), aiming to reach over 10,000 families.
